Summer CCD Forms 2012

IMPORTANT INSTRUCTIONS FOR COMPLETING SUMMER CCD REGISTRATIONS:

Registration for Summer CCD begins Monday, January 30, 2012. Forms may be downloaded from our website at stachurchbloomfield.com or may be completed in person at the Religious Education Office starting the 30th at 10:00 am Please do NOT drop off the forms until 10:00 am on the 30th or they will NOT be accepted. Please note that this program fills up very quickly and only completed registrations will be accepted. A completed registration package includes – a registration form, a behavior contract, and a volunteer sheet – all three forms must be handed in to begin processing the registration. If your child attended Summer CCD last year they are NOT automatically accepted into the program this summer. All completed registration forms MUST be dropped off ONLY in the Religious Education Office which is located in the basement of St. Thomas School. Registrations dropped off in the STA School Office or in the Parish Center will NOT be accepted. Please note that this program is for families that have belonged to the parish of St. Thomas the Apostle for at least one year. As always, volunteers are needed for all grades.
